Results of your personal blood tests are completely CONFIDENTIAL and
sent to YOU and ONLY to you unless you specifically request - in writing
- that we send a copy to your physician or other persons.

Health Insurance Portability and Accountability Act ( HIPAA)
agreement that, under federal law, we are
required to have you read, sign, and return to us before going for your blood
test.
